Or wanting to step up into a flexible and exciting new team?Our newly formed Physical Health Team in Buckinghamshire have an opportunity to welcome a new member.We support four Community Mental Health Teams (CMHTs) to look after the physical health needs of their patients who have Serious Mental Illness (SMI). We deliver specialised Clozapine and Lithium clinics, complete annual cardiometabolic assessments and high dose Antipsychotic monitoring. Alongside this we provide education, advice and assistance to Keyworkers.Physical health has had importance within our directorate for many years. We are currently reviewing our ways of working to be able to support the teams more efficiently. You will be part of delivering our well established Clozapine and Lithium Clinics and conduct physical health assessments for a variety of patients.We need you to have clear communication skills and be able to build a rapport with our patients and teams. Education is at the heart of what we do and we want you to be to have the knowledge and confidence to be able to give healthy living advice and be able to signpost/refer our patients to the required support.Main duties of the jobAs a part of our small but mighty team we want you to to have an interest in physical health and a passion for providing the best care to those with mental illness alongside a good working knowledge of mental health conditions and risk assessment.Phebotomy, ECGs and using Point of Care Testing will be a large part of your job and, we can help you brush up your skills.
