
Practice Nurse

- Lead and manage long-term conditions such as hypertension, diabetes, asthma, COPD, and cholesterol management.
- Provide care for elderly, frail, and housebound patients, including those in care homes.
- Conduct home visits as required.
- Support quality improvement and assurance initiatives within the PCN.
- Conduct chronic disease management within the scope of expertise.
- Provide immunisations, including:
- Routine influenza, pneumococcal, shingles vaccinations.
- Infant immunisation and travel health care vaccinations.
- Perform cervical smears, pregnancy tests, and contraceptive advice.
- Assist GPs with minor surgeries and coil fittings if required.
- Carry out ECGs, venepuncture, urinalysis, and wound care (including ulcer management and suture removal).
- Review and action pathology tests such as urine culture and swabs where appropriate.
- Chaperone and assist patients being examined by other clinicians.
- Leadership & Team Support
- Mentor and supervise any new-to-practice nurses, nursing associates, and HCAs.
- Work collaboratively with the wider practice team to enhance patient care.
- Promote public health and screening programs, including immunisations and cervical screening.
- Integrate population health management approaches to reduce health inequalities.
- Maintain accurate and up-to-date patient records.
- Ensure compliance with clinical protocols, including prescribing audits and reviews if qualified.
- Adhere to NMC revalidation requirements and practice within the NMC Code of Conduct.
- Follow infection control policies and ensure safe handling of specimens.
- Contribute to audits and service evaluations.
- Support the effective use of NHS resources and practice policies.
- Practice & Housekeeping Duties
- Ensure the cleanliness and tidiness of nursing and treatment rooms.
- Maintain and clean clinical equipment.
- Monitor and order medical stock, drugs, and vaccinations.
- Attend and contribute to practice meetings and training sessions.
- Comply with national infection control standards and health and safety guidelines.
- Ensure safe handling, storage, and transport of specimens.
- Identify risks in work activities and take necessary precautions.

- Previous primary care experience.
- Experience of NHS IT Systems EMIS.
- Good level of communication skills both verbal and written which can be tailored to a wide range of stakeholders.
- Good level of ICT knowledge and skills.
- Good organisational skills and time management.
- Professional, calm and efficient manner.
- Self-motivated, can work well independently and within a team.
- Works safely and effectively when working autonomously and in conjunction with other health care professionals.
- Hold more than one academic Level 6 diploma or postgraduate certification in long-term conditions care and/or public health initiatives.
- Registered with the NMC and maintains revalidation in line with NMC requirements.
- Working at Registered Nurse Level Practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework.
- Experience of clinical audit, evaluation research and implementation of evidence based practice.
- Experience working within a teaching/training environment.
- Understanding of national and local guidelines.
- Understand clinical governance and risk management.
- Evidence of post qualifying and continuing professional development.
- Spirometry training.
- Previous primary care experience.
