• Conduct comprehensive assessments and develop care plans tailored to individual patient needs.
• Provide specialist mental health services including triaging referrals and conducting duty assessments.
• Collaborate with a multidisciplinary team to ensure high-quality care under the Care Programme Approach framework.
• Keep detailed and organised records using the PARIS system, ensuring all information is accurate and up-to-date.
• Support patients and their families through the therapeutic process, offering advice and guidance as needed.

• Familiarity with the Care Programme Approach framework is highly desirable.
• Experience in triaging referrals and completing duty assessments is essential for this role.
• A minimum of six months of experience working at Band 5 level is required.
• Proficiency in using the PARIS system for detailed case documentation.
