- Provide expert management of long-term conditions, including:
- Diabetes
- Asthma
- Chronic Obstructive Pulmonary Disease (COPD)
- Hypertension
- Heart Failure
- Conduct medication reviews and prescribe safely and effectively in line with current evidence-based guidelines.
- Request and interpret diagnostic tests such as blood investigations, ECGs, and X-rays.
- Deliver health promotion advice and support patients in self-management and lifestyle modification.
- Triage and manage same-day urgent presentations appropriately and efficiently.
- Refer patients to secondary care, community services, or other appropriate agencies where necessary.

- Engage in practice meetings, MDT discussions, and strategic planning initiatives.
- Provide clinical leadership, support, and supervision to nursing and healthcare support staff.
- Contribute to the induction, teaching, and ongoing professional development of new staff and students.
- Act as a role model for evidence-based practice and lifelong learning.
- Champion a culture of safety, collaboration, and accountability within the clinical team.
- Maintain accurate, legible, and timely patient records in line with professional and legal standards.

- The role involves face-to-face, telephone, and digital consultations.
- Occasional home visits may be required.
- The postholder will be expected to participate in extended hours or weekend clinics as needed.
