
PCN Social Prescribing Link Worker

- Significant experience of working with vulnerable people who may have multiple needs and complex lives.
- Experience of person-centred service delivery in a community or clinical
- setting e.g. advocacy, support work, counselling or social work.
- The ability to work autonomously and to plan, prioritise work under pressure and adapt to new models of working.
- Extensive experience of using the Microsoft Office suite.
- Experience of working collaboratively and in partnership with statutory and VCSE sector organisations.
- Relevant qualification in Community Development, Health or Social Care, to degree level or equivalent experience.
- Flexible approach to working hours; able and willing to work outside of normal office hours, with appropriate notice, to accommodate the demands of the service.
- A valid driving licence and use of a car for work-related travel.
- High degree of empathy,understanding, honesty and integrity.
- Positive work ethic: professional,enthusiastic, dependable, adaptable,conscientious.
- Ability to work flexibly and effectively as part of a team.
- Proactive approach to personal development and the updating of skills and knowledge
- An understanding of social prescribing and the value of VCSE sector services to support people with their physical and mental health

- Strong IT skills including spreadsheets,databases and the Microsoft Office suite.
- Demonstrate a good understanding of Social Prescribing, personalised care and person-centred approaches used to support people in the community.
- Experience of working with an asset-based / person-centred approach.
- Excellent holistic assessment skills, experience of providing empowering
- support to adults in a planned and structured way to improve health and well-being outcomes.
- Ability to analyse issues and reach creative solutions with people.
- Ability to work collaboratively and in partnership with individuals, referrers, providers and commissioners to include primary care, secondary care voluntary sector and the local authority.
- Able to act with confidentiality, discretion and regard to sensitivity of information.
- Excellent communication and negotiation skills to build and nurture relationships at a personal and community level. The ability to relate well to individuals of all ages, of all backgrounds
- Knowledge and understanding of multiple complex needs, safeguarding and how to respond to risks / vulnerabilities.
- A strong understanding of safeguarding for vulnerable adults and ability to implement relevant policies and procedures.
- Strong interpersonal skills, ability to communicate with stakeholders across multidisciplinary teams.
- An understanding of motivation and behavioural change.
