What you will do:
  • Work alongside senior physiotherapists, Continuing Healthcare nurses, domiciliary care staff and care home teams to facilitate a seamless discharge process for patients.
  • Provide tailored physiotherapy assessments for patients recently discharged from the hospital, ensuring a smooth transition to appropriate care settings.
  • Deliver high-quality physiotherapy plans to aid recovery in both home and care settings, enabling patients to regain independence and achieve their goals.
  • Offer advice, guidance, and education to patients, their families, and caregivers to ensure understanding and confidence in care after discharge.
  • Collaborate with the wider multidisciplinary team to problem-solve challenges and achieve the best outcomes for patients.
  • Effectively maintain and update patient records in line with HCPC standards.
  • Stay proactive in identifying ways to improve the service and optimise results for individuals being cared for.
