- Collaborate with the Leaving Care Team to plan effective pathways and support for young people transitioning into adulthood
- Develop and implement care and pathway plans tailored to the needs of each young person
- Build and maintain strong relationships with partner agencies to enhance the support and services young people receive
- Monitor and review the progress of each care leaver, adjusting support plans as necessary
