We have an exciting opportunity for a Prescribing Paramedic Practitioner to join our team across our 5 practices. This is a dynamic, patient-facing role focused on providing urgent, same-day care and home visits within our community. You will work independently while collaborating closely with a diverse clinical team that includes GPs, nurses, pharmacists, social prescribers, and other healthcare professionals. As part of this role, you will also maintain accurate patient records in line with legislation and local policies, ensure compliance with GDPR and confidentiality requirements, and use clinical systems and software effectively to support patient care and audit processes.Main duties of the job
  • Deliver safe, high-quality patient care independently, carrying out advanced clinical assessments, formulating diagnoses, providing treatment, and arranging onward referrals as required.
  • Conduct in-person appointments and home visits within the PCNs catchment area.
  • Assess and manage patients presenting with undifferentiated or undiagnosed minor illnesses and injuries, with senior clinical support available when necessary.
  • Carry out specialist checks and clinical procedures such as ECGs, phlebotomy, and swabs, and analyse the results to inform care.
  • Contribute to proactive care planning and lead local health projects, including monitoring blood pressure and diabetes risk factors.
  • Offer a responsive alternative for urgent and same-day home visit needs within the community.
  • Work in close partnership with GPs and the wider PCN team, receiving and making appropriate referrals to ensure seamless patient care.
  • Promote patient understanding of their conditions, providing clear guidance to encourage self-management and healthy living.
  • Keep thorough, timely clinical documentation in compliance with GDPR and best practice in clinical governance.
  • Triage and address health concerns effectively, taking prompt action in complex, urgent, or emergency situations to deliver safe, immediate care.
  • Treat and support patients with both short-term and long-term conditions, combining medication and non-pharmacological approaches as appropriate.

  • Registered paramedic with current HCPC registration - UK only.
  • Qualified in Phlebotomy.
  • Advanced Clinical Assessment, Reasoning & Decision-Making skills.
  • Excellent clinical practical skills and confidence in assessing, planning, treating and referring patients.
  • Experience working to clinical protocols and guidelines.
  • Strong verbal and non-verbal communication skills.
  • Ability to make clear clinical decisions and communicate them effectively.
  • Understanding of scope of practice within HCPC guidelines.
  • Experience working within a multi-disciplinary team.
  • Ability to work flexibly, independently, and collaboratively to meet service needs.
  • Full UK driving licence and access to a vehicle for work purposes essential. Business mileage will be reimbursed in line with policy.
Desirable
  • Previous experience working in a primary care or community care setting.
  • Experience working across organisational boundaries within health and social care.
  • Experience using SystemOne.
