You’ll build strong relationships with carers & healthcare professionals, including dietitians, GPs, and local nurses.This field-based position supports Adult Patients in the Brighton Area. To be considered for this role, you’ll need to be a Registered Nurse, (RGN, RSCN or RNLD), with at least 2 years’ experience and living in Brighton. You’ll also need a full UK Driving License; we’ll provide a company car, fuel card and a lunch allowance when you’re in the field.Role Focus:To develop and deliver comprehensive training to patients, carers, and HCPs to support safe discharges from the hospital, as well as ongoing training for quality care at home.Complete regular patient reviews and be on hand to support with questions, issues and troubleshooting requests.ABOUT YOUYou'll play a pivotal role in nurturing patient & HCP relationships.
