Contract role, Hospital Discharge TeamPertemps are recruiting for experienced Adult's Social Worker in the North Lincolnshire area.
This is working as part of the Single Point of Access Team which will include home visits and completing Care Act Assessments, working within the hospital integrated discharge team based at Scunthorpe General Hospital.
Completion of MCA/BI as necessary .This is an agency post and Pertemps can offer you:-
