Successful applicants will be invited to attend a Selection Day at Carew House, Bodmin on 19th September 2025 (morning or afternoon session - to be confirmed).We are offering an exciting opportunity for a motivated and compassionate Community Health Care Assistant to join our Coastal Community Nursing Team in Cornwall.This role involves delivering high-quality nursing care to patients in their own homes, including those receiving end-of-life care. Duties may include wound care, venepuncture, catheter care, pressure area management, and personal care.We value integrated, patient-centred care and are committed to supporting your professional development. Previous care experience is essential.The role requires the ability to travel independently across the Coastal PCN area. Travel time is included in your shift, and mileage is reimbursed at Agenda for Change rates.This is a community-based position, covering a wide geographical area from 8am-10pm with 7.5-hour shifts.Please note: If you require sponsorship, please check the eligibility criteria for a Certificate of Sponsorship on the gov.uk website before applying.Main duties of the jobTo carry out assigned tasks while regularly working without direct supervision involving direct care to patients and carers to support the registered nurse.To work as a member of the community nursing team within a given geographical area.The District Nursing Team work on a flexible rota shift system covering the period from 8am to 10pm which includes an on call system offering a high standard of service.About usWe're an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly.
