Northern Case Management (NCM) was established in 1997 to provide support to people with acquired neurological damage (i.e. brain, spinal cord injury) and empower them to develop their full potential and enrich their lifestyle through providing a comprehensive client focused service.NCM work closely with the client and their family or representative to tailor support packages for clients in their own homes and in the community.NCM continues to meet the increasing requirement for case management services and now boasts a multi-disciplined team of 8 case managers plus support staff and manages over 200 support workers on behalf of Clients.What you'll be working:All applicants must hold a full UK driving licenseFemale applicants onlyOur client is a young girl aged 13 with Cerebral Palsy she is looking for passionate, fun loving, caring support staff to join her support team supporting her in her home, school and community.She suffered a brain injury at birth, she has a permanent PEG (feeds and medication needs). She also requires support with all Activities of Daily Living. Experience preferred but not essential as you will work as part of a team and will receive all required training including regular clinical supervision.She attends school and to meet her needs and her family's the shifts are split.Monday to Friday 08.00 - 10.00 (to support with getting to school).Monday to Friday 14.00 - 20.00 (meal preparation, socialisation and preparing for bed); Saturday & Sunday 08.00 - 20.00 (to support with access to the community).Additional hours during school holidays are required, from 08.00 till 20.00You will be required to drive our client's wheelchair adapted vehicle- to escort her to and from school/in and around the community.What you'll be doing:
  • To establish and maintain a trusting, therapeutic, working relationship with client, family & Case Manager
  • To support the client in line with the Care Plan guidelines
  • To promote, monitor and maintain health, safety and security in the workplace
  • To promote effective communication with all parties.
  • To contribute to the protection of individuals from abuse.
  • To liaise with other agencies when required
  • To complete daily records as advised including; time sheets, daily records, care plan summary and critical incidents, petty cash etc
  • To attend supervision, team meetings and other meetings with the employer and other persons as nominated by the employer
  • To attend training sessions as arranged
  • To support the client to attend and participate in a range of community activities.
  • To support the client to attend hydrotherapy if required.
  • To carry out therapy practise sessions as advised by treating professionals
General
  • Observe and operate at all times in accordance with the Company's policies and procedures.
  • Provide a flexible working pattern, to ensure the smooth operation of service, including working out of core hours, weekends and public holidays.
  • To undertake any other duties appropriate and as reasonably required, to meet the changing needs of the service.
  • Ensuring service user needs and business objectives are met.
What you'll have:
  • Have some experience of neurological conditions and working in a community setting.
  • Experience of working with client with complex needs is desirable
  • Ability to work as part of a team and as an individual.
  • Self-confidence and able to manage challenging situations.
  • Excellent interpersonal, verbal and written communication skills.
  • Strong organisational, time management and planning skills, with a commitment to self-management and continuous personal development.
  • IT literate. Use of MS Word, Excel and Outlook.
  • Flexible, professional and positive approach to multi-site working.
  • Proven reliability in terms of full attendance and timekeeping.
  • Full UK driving licence.
