What you will do:
  • Conduct assessments in line with the Care Act to identify needs and develop support plans for individuals aged 18 to 25 with learning disabilities.
  • Work collaboratively with professionals, service users, and their families to ensure comprehensive support and service delivery.
  • Provide guidance and advocacy on the Mental Capacity Act, ensuring legal compliance and safeguarding practices are upheld.
  • Maintain accurate case records and prepare detailed reports as required, demonstrating accountability and clarity in all communications.
  • Undertake regular reviews of care plans to adapt support to individuals’ changing needs.
  • Engage in multi-disciplinary meetings to share insights and develop holistic care strategies.
