- Support and guide the Community Connections Primary Care Link Worker, ensuring comprehensive care that ties physical health considerations into mental health treatment.
- Develop training modules to upskill the Primary Care Team in the management of mental and physical health integration, bolstering service quality.
- Professional Qualification in Mental Health Nursing, Occupational Therapy, or Social Work.
- NHS experience and familiarity with SystemOne and EMIS.
- Experience with evidence-based mental health interventions for various conditions.
