We have an exciting opportunity for a Healthcare Support Worker to become part of our dedicated team in Devizes, working 20 hours and 30 minutes per week.At HCRG, were passionate about expanding and enhancing our community services, aligning with the NHS Long Term Plan to transform out of hospital care. Were strengthening our existing community teams by increasing response capacity and introducing a 2-hour crisis response element, ensuring high-quality care around the clock. Now, we're extending our nursing services across Wiltshire! Our overnight nursing service will provide essential care when patients need it most.Main duties of the jobTo implement treatment by following care plan.To keep the patient at the centre of his or her care, ensuring patient and carer participation in decision making.To undertake the daily care and support of delegated patients.Deliver treatment/care within a community setting as specified in a written care plan, under the guidance of a registered practitioner.Wherever possible to avoid hospital admission and reduce inpatient length of stay to ensure that patient receive the appropriate care in the most appropriate setting.Responsibility for the day-to-day care of patients including monitoring.Support the taking of medication under the authorisation of the qualified practitioner.To have a good knowledge of infection control, to reduce risk of health care acquired infections.To enable patients to reach their optimum level of independence.Please see our JD for the full listAbout usAbout the CompanyWe change lives by transforming health and care.Established in 2006, we are one of the UKs leading independent providers of community health and care services, working with health and care commissioners and communities to transform services with a focus on experience, efficiency and improved outcomes.
