- Registered Nurse (current NMC registration).
- Evidence of continued professional development.
- Experience of working within a clinical environment.
- Experience of managing a range of long-term conditions (or willingness to undertake training).
- Experience in health promotion and patient education.
- Knowledge of current nursing practice, clinical guidelines, and professional standards.
- Ability to assess, plan, implement, and evaluate care.
- Proficient in core nursing skills (e.g., wound care, immunisations, cervical cytology or willingness to train).
- Excellent communication and interpersonal skills, with the ability to build rapport with patients and colleagues.
- Ability to work independently and as part of a multidisciplinary team.
- Strong organisational and time-management skills.
- Good IT skills, with the ability to use clinical systems and maintain accurate records.
- Commitment to high-quality, patient-centred care.
- Flexible, adaptable, and proactive approach to work.
- Professional, reliable, and able to maintain confidentiality at all times.
- Commitment to ongoing professional development and training.
- Post-registration qualification(s) in chronic disease management (e.g., asthma, COPD, diabetes, cardiovascular disease).
- Cervical cytology training (up to date).
- Immunisation and vaccination training (adult and child).
- Contraception and womens health training.
- Previous experience working in a general practice or primary care setting.
- Experience working within a multidisciplinary team.
- Experience of contributing to Quality and Outcomes Framework (QOF) targets and Enhanced Services.
- Understanding of safeguarding (adult and child protection) procedures.
- Familiarity with clinical systems (e.g., EMIS).
- Ability to support training and mentoring of junior staff or students.
- Interest in service development and innovation.
- Willingness to undertake further training to meet the needs of the practice.
- Ability to adapt positively to change in a fast-paced environment.
