To be able to work as a member of a team, to provide support, and give time to individuals referred to PCDU. The team will work with individuals who require short term immediate support to allow them to engage with support networks in their community, the support could be around employment, housing , financial issues, interpersonal relationships or other social issues. The primary focus of the post will be short term work utilising support from ordinary resources within the community and urgent care.Main duties of the jobTo assist Clinicians in the assessment of health and social need of individuals referred to PCDU.Provide emotional support to patients, relative and carersUnder the direction of clinical staff, the post holder will assist individuals to access community support to enable them to leave the PCDU and used appropriate community services to meet their needs.To assist and engage in practical activities with individuals to enable them to live as independently as possible and promote social inclusion.
