Responsibilities:
  • Developing and implementing comprehensive care plans that prioritize the child's welfare, stability, and long-term outcomes. This includes facilitating timely permanency planning and coordinating transitions to adulthood for young people aged 16-18.
  • Conducting consistent visits to children in care to assess their well-being, ensure their needs are being met, and maintain strong relationships.
Working closely with health professionals, educators, carers, and other stakeholders to provide holistic support tailored to each child's unique needs. * Ensuring that the voices of children and young people are heard and considered in all decisions affecting them. This includes facilitating access to advocacy services and encouraging participation in forums like the Children in Care Council.
  • Promoting educational attainment and addressing health needs by coordinating with schools and healthcare providers to support the child's development.
  • Managing legal processes related to care proceedings, ensuring compliance with statutory requirements, and maintaining accurate records of all interventions and decisions.
Requirements:
  • Social Work Degree or equivalent
  • Current registration with Social Work England
  • Enhanced DBS
