- Conduct comprehensive whole-family assessments, maintaining professional boundaries and establishing realistic goals.
- Coordinate and deliver targeted support for families, emphasising the well-being of children and young people in decisions.
- Lead in priority areas such as Domestic Violence, Parental Mental Health, Housing, or SEND.
- Identify and work with multi-agency partnerships to develop SMART plans in collaboration with families.
- Record all assessments and interventions meticulously on case management systems.
- Respond to significant events promptly and escalate matters appropriately to management.
- Sustain positive changes, increased independence and reduced risk factors for families.
- Promote a culture of partnership and innovation within the team and service.
- To thrive in this position, you must have a Degree or equivalent in Social Work, a current Social Work England registration, and substantial frontline experience in social services within the UK.
