We are recruiting for experienced Social Worker to work in a Discharge to Assess Team in the London Borough of Tower Hamlets.
Social Worker required for Discharge to Assess in Initial Assessment. Discharge to assess supports residents who have been discharged from the hospital with a package of care, where they previously did not have statutory support in place. To complete care and support assessments, working with residents, their support networks and other professionals.
