Social Worker - South Community Adult Social Care Team Bristol City Council | Full-Time | About the Role An exciting opportunity has arisen for an experienced Social Worker to join the South Community Adult Social Care Team at Bristol City Council. This is a fast-paced and rewarding role where you will support adults in the community by completing assessments, reviews, and support planning under the Care Act 2014 . You will work in a multi-disciplinary environment , collaborating with health, housing, and voluntary organisations to deliver person-centred and strength-based support to Bristol's diverse communities. Key Responsibilities Undertake Care Act assessments, carer assessments, and reviews , ensuring individuals receive the right level of support. Develop support plans that promote independence, choice, and well-being. Complete mental capacity assessments and best interest decisions where required. Lead on safeguarding enquiries , working in line with safeguarding policies and procedures. Work closely with health, housing, and voluntary sector partners to ensure effective multi-agency support. Maintain accurate and timely records in line with statutory requirements. What We're Looking For Qualified Social Worker , registered with Social Work England . Experience in adult social care , including Care Act assessments and safeguarding. Knowledge of mental capacity assessments and best interest decision-making . Strong communication and interpersonal skills, with the ability to work collaboratively. A commitment to strength-based and person-centred practice .
