  • Assess and treat adult patients with neurological conditions, with a focus on intervention for acquired communication and swallowing difficulties in the community setting
  • Provide support to patients in their homes, care homes, and other community settings alongside Sutton GPs
  • Manage patients with conditions like stroke, head injury, Parkinson's disease, multiple sclerosis, and other progressive conditions
  • Work autonomously in managing dysphagia, ensuring comprehensive care delivery
  • Collaborate effectively within the Multidisciplinary Team (MDT) to facilitate holistic patient care
  • Complete community visits around the borough of Sutton, ensuring the best possible support for patients
  • Regularly document assessments and interventions using EMIS, provided you have a NHS SMART card
Essential requirements:
  • Proven experience working with patients with neurological conditions
  • Experience in rehabilitation and managing long-term conditions
  • Dysphagia trained with the ability to work autonomously
  • Community experience with adults and older adults
  • Proficiency in MDT working
  • Must have a car and a valid driving licence
Desirable:
  • EMIS trained
  • Able to claim mileage, although parking permits are not available
