1.Post is a 21 hour per week Support Worker
Working to a six-week rolling rota subject to change to meet needs of the service.
- There is a requirement to work shifts day and evenings also weekends and Bank Holidays.
About you:
- We are looking for someone who has a positive attitude and a passion for working with people who have a Learning disability.
- Be able to deliver a service that is person centred has a strong focus on positive outcomes and an increase in people’s wellbeing.
- To have an outgoing and friendly attitude with a positive outlook and a desire to meet all required challenges and aspects of the role.
- To be able to think outside of the box and find creative solutions.
- To always embrace and deliver the corporate values in everything we do these being.
- To have the skills and knowledge on how to treat people with learning
- To be the holder of a full clean driving licence
- Supporting people with Learning disabilities to develop their skills and independence by demonstrating and modelling activities.
- Working alongside a dedicated staff team and other professionals in the service users’ own home and other required venues.
- Undertaking practical tasks and supporting people with their personal care, domestic activities and socially in the community including driving service users to activities and appointments.
- Supporting service users to build and maintain family and friendship links and to reach goals and achieve personal outcomes.
- Participating constructively in regular staff supervision and appraisal sessions.
- Contribute to written assessments and Care Plans and attend Care Planning meetings compiling and keeping detailed records and reports.
- Participating/attending required training to develop, grow and gain skills to enhance the role.
