Neston & Willaston Primary Care Network (PCN) have an exciting opportunity for a motivated Clinical Pharmacist to join our expanding team.The clinical pharmacy team is integral within the PCN to improve quality of care and patient safety.The Clinical Pharmacist will be a highly motivated individual who has completed their CPPE pathway and ideally be a non-medical prescriber or working towards this.Main duties of the job
To support the PCN to ensure the safe delivery of repeat prescribing.
to support patients and practice staff to optimise medications by promoting good practice inline with clinical and operational guidelines
To support the PCN with achieving the prescribing scheme
To reduce medication waste
To support pharmacy technicians with medicine reconciliations
To support all members of the General Practice teams.
About usNeston & Willaston PCN was established in 2019 and is one of the smallest PCN's in the country, responsible for approximately 21,000 patients.The Network is a collaboration between Neston Surgery, Neston Medical Centre and Willaston Surgery. The PCN work closely within the community, aiming to create a healthier future for our patient population.The PCN have a dedicated team who are responsible for planning, sourcing funding and implementing these services.The PCN have well-established roles including a Visiting Paramedic Service, Physiotherapy, Social Prescribing Link Workers and a Clinical Pharmacy team.Our Vision is to:
