What you will do:– Facilitate and manage safe hospital discharges for service users, enabling a smooth transition back home.
– Conduct comprehensive assessments of individual needs, focusing on functional abilities, moving and handling, as well as housing.
– Prescribe suitable equipment and recommend minor home adaptations to support individuals in maintaining independence.
