Directorate: Adult Care ServicesAbout the teamThe Health Liaison Team supports people with Learning Disabilities who are Admitted to hospital for planned or emergency admissionAbout the roleYou will be working to support clinical colleagues to understand and implement reasonable adjustments to reduce the risk of health inequalities. You will be co-delivering training to clinical staff with Experts by Experience through structured and informal sessions, along with supporting Student Nurses as part of the wider Health Liaison Team. We would encourage people who are passionate and creative about individual approaches to take on this hugely rewarding role.About youEssential Criteria
- Possesses a full UK driving licence
- Demonstrated experience supporting and working with individuals with learning disabilities within community settings
- Familiarity with acute hospital environments and associated processes
- Skilled in collaborating with a range of professionals across disciplines
- Strong team player with experience contributing to multidisciplinary teams
