What you will do:
  • Conduct Care Act assessments and develop personalised support plans for adults aged 18+ with long-term physical disabilities, as well as those aged 65 and over.
  • Collaborate closely with Occupational Therapists, Social Care Co-ordinators, and partner agencies to promote independence and well-being.
  • Apply your strong understanding of adult safeguarding processes to ensure the safety and dignity of the individuals you support.
  • Undertake Mental Capacity Assessments with confidence and sensitivity.
  • Engage in court work, including applications to the Court of Protection, leveraging your professional expertise in a legal context.
