- Develop and implement a comprehensive self-management programme for service users.
- Coordinate the full range of recommended psychological, pharmacological, social, and occupational interventions.
- Manage a caseload and lead care coordination, including assessment, care planning, and working collaboratively with other professionals.
- Promote recovery by supporting service users in adopting strategies to enhance their independence and wellbeing.
- Undertake regular CPA reviews and liaise effectively with inpatient services and other interface services such as Addictions and CAMHS services.
- Administer and monitor medication while educating service users on its effects and benefits.
- Engage with a diverse client base, addressing cultural and ethnic differences to enhance care engagement.
- Participate in audits and quality improvement projects to uphold the highest standards of care.
- NMC Registration as a Registered Mental Health Nurse and/or Registered Learning Disabilities Nurse
- Clinical experience in mental health, including managing a caseload
- Ability to apply evidence-based practice and reflect using clinical supervision
- Proven CPD (Continuing Professional Development)
- Mentorship/preceptorship experience
- Understanding of clinical governance, audit, and relevant professional legislation
- Experience with Care Coordination under CPA
- Experience of inter-agency working and community-based care
