- To offer a high-quality service to the patients by providing smooth-running efficient support to the general practitioners and clinical teams at BMG.
- Support patient experience by translating or reiterating information from the GP or other lead clinicians involved in LTC care.
- Provide pre-assessment actions to support LTC management.
- Processing T-Quest requests.
- Organise and facilitate group consultations/ clinics.
- Work to improve access and reduce unnecessary contacts with clinicians. Completion of frequent attenders searches to identify patients who are attending multiple times outside of LTC recall, discuss with LTC clinical team, provide contact to improve efficiency and continuity, signposting where appropriate. Monitor and review to improve efficiency and report on outcomes.
- Completion of referral forms such as ECG/ ECHO/ APBM/ Weight management/ Pulmonary function tests/ CDC and other providers for booking of pre-requisite investigations.
- Communicate sensitive information, test results, explain medication changes and arrange appropriate follow up with patients.
- Gather further information from patients and third parties when required, ensuring the information is recorded appropriately in patient records.
- Act as a chaperone in accordance with practice chaperone policy, including record keeping where appropriate.
- Optimise appointments by speaking with patients who may benefit from reasonable adjustments or further information about their LTC monitoring.
- Review lists of non-responders to LTC appointments and contact them, after discussion, either making a booking for them or excluding as appropriate.
- Identify high risk multimorbidity patients and non-attenders. Highlight and discuss these patients with LTC leads or in MDT Micro team meetings and work alongside patients for improvement.
- Produce reports to inform Quality Improvement for QOF and report on outcomes.
- Scheduling meetings, collating information, and producing agendas, take minutes, produce action sheets, complete non-clinical actions etc e.g MDT (Multi-disciplinary team) meetings.
- Take part in audit cycles as required.
- Raise any potential issues with line manager.
