You will lead on creating and reviewing individualised care plans, working closely with residents, their families, and other healthcare professionals to ensure needs are met in a safe, respectful, and person-centred way. You will organise and guide your team at the start of each shift, provide clinical oversight throughout the day, and ensure handovers are effective and well-communicated. You will administer medication safely, maintain accurate documentation, and monitor residents’ health and wellbeing through regular observations. You will champion resident independence, respond swiftly to emergencies, support nutritional needs, and encourage social engagement and community involvement. You will also be a key contact for families, handling concerns with empathy and professionalism.About You
- You are a Registered Mental Health Nurse (RMN) with an active NMC pin
- You are passionate about delivering high-quality care and improving the lives of others
- You bring warmth, empathy, and calm professionalism to your role
- You communicate clearly and kindly with residents, families, and colleagues
- Experience in a care home is welcomed, though not essential
