- Supportive team environment: Collaborate with a team that values your input and encourages growth.What you will do:- Carry out initial screening and assessment of all service users, undertaking intensive short-term treatment where appropriate.
- Manage a caseload of clients with complex Occupational Therapy needs, applying evidence-based and client-centred principles to assess, plan, implement, and evaluate interventions.
- Perform case formulations post-assessment to highlight occupational strengths and limitations, informing intervention plans.
- Assess and prescribe appropriate equipment and aids to ensure patients’ safety and independence, offering advice to staff, families, and carers when needed.
- Monitor and evaluate treatment plans, collaborating with teams both within the Trust and third sector organisations as appropriate.
- Document and record all assessments accurately, adhering to organisational and professional guidelines.
- Respect and consider the needs and preferences of service users, their families, and significant persons in care planning.
- Involve patients in goal setting and treatment proposals, obtaining valid informed consent while working within the legal framework.
- Apply the Model of Human Occupation (MOHO) and other standardised assessment tools relevant to the clinical setting.
- Collaborate with service users to identify Occupational Therapy goals as part of the overall recovery plan.
- Deliver interventions aligned with Trust standards and values.
- Provide feedback to the MDT regarding occupational performance, informing ongoing assessment and therapeutic intervention.
- Offer specialist support and education to service users and carers on occupational therapy aspects, such as equipment fitting and rehabilitation programmes.
- Follow the RCOT Code of Ethics and Professional Conduct alongside national and Trust policies.
- Reflect on your practice and performance using professional and operational supervision and appraisal effectively.
- Function as an effective MDT member, supporting local ward policies and participating in core team delivery, including PEACE interventions.
- Participate physically in manual handling of patients and assist staff in adhering to safe moving and handling protocols.Requirements:- HCPC registered
