What you will do:
  • Collaborate within an integrated network team to facilitate patients' smooth discharge from hospital settings
  • Implement strategies to prevent patient admissions through effective community-based interventions
  • Undertake tailored therapeutic interventions aimed at rehabilitation and recovery within patients' homes
  • Plan and deliver therapy as part of Pathway 1 and Urgent Community response initiatives
  • Utilise your skills and expertise to support patients in their journey to independence and improved well-being
