Job descriptionJob responsibilitiesClinicalFrequently involves direct patient contact via face to face, video or telephone. May include domiciliary or care home visits.Competently run patient clinics, organising own workload and processes involved to ensure timely patient follow-up and medicine optimisation.Participate in and lead on long term condition management plans, always considering a holistic approach within your area of competence.Complete structured medication reviews (SMRs).Prior to a review ensure any necessary monitoring has been established / encouraged or arrange it. As competence grows this will involve ordering and then later reviewing blood tests.Confidently review and, in collaboration with other HCPs, competently make changes to therapy if required. Be prepared to refer for additional care, information or support as necessary.Support in improving patient understanding and education. This could involve group sessions or the production of health promotional information and materials.Address day to day issues raised by patients / staff or other HCPs in relation to medicine management. - Mandatory registration with General Pharmaceutical Council (GPhC)
- Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience.
- Masters degree in pharmacy (MPharm) (or equivalent)
- Completion of CPPE PCPEP (or equivalent)
- Independent prescriber or working towards/intent of gaining an independent prescribing qualification
- In depth therapeutic and clinical knowledge and understanding of the principles of evidence- based healthcare.
- Excellent interpersonal, influencing and negotiating skills.
- Excellent written and verbal communication skills
- Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g., patients)
- Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
- Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate.
- Computer literate with an ability to use the required GP clinical systems (desirable) and Microsoft office packages (essential)
- Able to analyse and interpret prescribing data.
- Has attention to detail, able to work accurately, identifying errors quickly and easily.
- Able to effectively manage allocated resources.
- Has a planned and organised approach with an ability to prioritise their own workload to meet strict deadlines.
- Able to think analytically; anticipating obstacles and thinking ahead; using analytical techniques to draw logical solutions to problems.
- Excellent communication skills, verbal and written, with the ability to adjust communication style and content to suit the audience.
