We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included.The shift pattern is 09.00-17.00 (28 hours).This is a Fixed term post for 12 months.(Please note the salary is Pro Rata) for part time hours.An exciting opportunity has arisen for a fixed term Care Home Liaison Nurse B6 within the NE Older Peoples CMHT. The successful candidate will work collaboratively with patients, carers, the wider multi-disciplinary team and third sector organisations. A strength based, person centred approach will be essential to assessment and formulation of care plans to meet the identified needs of the patient. Implementation of the Physical Health Care policy will be a critical role, providing monitoring in line with current GG&C guidelines to ensure timely and accurate record keeping.A key role will be providing support and guidance to care home staff and engaging in regular MDT discussions, along with the administration of depot medication and regular reviews of antipsychotic medication according to GG&C policy and guidelines. The role includes support and supervision to student nurses and junior staff and being part of the desk based CMHT duty rota.
