Yes, great then we may have the role for you.A fantastic and highly rewarding opportunity has arisen for a Community Inclusion Facilitator to join our enthusiastic and passionate colleagues in Sutton. We have 2 part time (20 hours per week) permanent contracts.As a Community Inclusion Facilitator you will be a valued colleague within a team, you will be flexible and committed to supporting people. Your role is responsible for developing and offering a range of responsive direct individual and group activities to people with a learning disability accessing the Me Time Programme.Key Responsibilities:
  • Work in a way that enables the people being supported to make decisions and choices and encourage trying new opportunities in line with the life each person chooses to lead.
  • Ensure the plans we put in place for the people we support are person centred and the person is fully involved in all aspect of the planning and review process.
  • Being able to be creative, dynamic, and adaptable to potential challenging scenarios in relation to the needs of the people we support.
  • Ensuring the safety and wellbeing of people being supported, monitor each person's progress and report on the impact that the programme has on the lives of people.
  • Complete all recording requirements in a timely manner and ensure high quality reporting and case studies.
  • Communicate with the people being supported with dignity and respect.
  • To be part of a team, supporting colleagues to build community networks, sharing contacts and connections Raises concerns using the appropriate channels, particularly in relation to safeguarding.
  • To be flexibility and can work from different locations and projects within the community.
