At The Forward Trust, we are committed to supporting individuals in their recovery from substance misuse and mental health challenges. We’re currently recruiting a Health & Wellbeing Practitioner to join our dedicated team at HMP Coldingley, a closed Category C male prison in Surrey.As part of an integrated healthcare framework, you’ll work alongside a multi-disciplinary team to deliver a mix of clinical and psychosocial interventions to adult male prisoners. This includes individuals presenting with low-level mental health needs and/or substance misuse issues.You will be responsible for:
Managing a caseload and delivering tailored recovery-focused interventions
Conducting assessments and co-producing SMART care plans with service users
Facilitating structured 1:1 sessions and group programmes
Providing harm reduction advice and practical recovery support
Supporting release planning and ensuring through-care into community services
Performing second signatory duties as needed
Maintaining accurate, up-to-date case records in line with service KPIs
This is a 7-day-a-week service, so weekend working will be required on a rota basis.What We’re Looking ForWe’re seeking a proactive, compassionate professional who brings the following essential skills and experience:
