We are looking for an experienced specialist health care professional who is self-motivated, dynamic and forward thinking to support patients with lipid disorders and endocrine conditions within Cwm Taf Morgannwg University Health Board.The Lipid/Endocrine Specialist Nurse will act as an expert resource providing specialist advice and support innovation, improved access, and high-quality patient care.The post holder will work collaboratively with the Diabetes Team, ensuring patient care is delivered to the highest standard, utilising research and evidence based practice in line with local and national policies, professional standards and guidance.Main duties of the job
  • Offer specialist advice on lipid and endocrine care to colleagues and develop professional networks locally and nationally.
  • Provide empathetic support to patients and carers, often dealing with anxiety, chronic illness, or complex emotional needs.
  • Share clinical findings and treatment plans clearly through verbal updates, reports and letters.
  • Represent the service in team and professional meetings, promoting coordinated, multidisciplinary care.
  • Support and evaluate service development projects and health improvement programmes

  • Registered Nurse
  • Degree or equivalent skills, knowledge and experience with specialist qualification in Diabetes or Endocrine care or equivalent knowledge.
  • In-depth understanding of chronic condition management.
  • Knowledge of the Vulnerable Adult and Child Protection procedures and their application in practice.
  • Risk management knowledge and involvement including incident reporting and infection prevention and control.
  • Evidence of ongoing Continuing Professional Development.
Desirable
  • Mentorship Qualification.
  • Independent prescribing qualification or willingness to work towards.
ExperienceEssential
  • Significant post registration clinical experience within the secondary or community setting.
  • Experience of teaching others within the clinical field with supporting evidence of involvement in teaching/presentation sessions.
  • Experience of participating in clinical audit.
  • Experience of working alongside multi-disciplinary teams and other disciplines.
  • Experience of liaising with a wide range of health care professionals, social services, and other organisations such as the Independent Care Sector.
  • Experience of supervising staff and managing resources.
