• Provide assertive outreach and in-reach services across a variety of settings, including streets, hostels, and day centres.
• Undertake comprehensive assessments of clients’ needs and develop personalised recovery and support plans.
• Deliver harm reduction advice and interventions, including safer injecting advice and overdose prevention.
• Collaborate closely with agencies like housing services, mental health, probation, and primary care.
• Support clients in accessing treatment services, stabilisation programmes, and housing support.
• Maintain accurate case records using internal systems and contribute to reporting and monitoring requirements.
• Identify and escalate safeguarding concerns appropriately.
• Promote service user involvement and co-production in recovery planning.
