
Community Mental Health Nurse - Named Worker Role

- Acting as a Named Worker
- Comprehensive assessment of new referrals
- Carrying a varied caseload defined by need
- Contributing to a duty system on a rota basis
- Delivering supervision to more junior members of staff
- Contribute to Multidisciplinary service development
- Triage and determine urgency of new referrals to the service
- Carry out comprehensive assessments of an individuals health and social care needs in a variety of settings and agree the most appropriate course of action. This includes first presentation to service.
- To assess, evaluate and manage clinical risk
- To formulate a plan of care based on identified need
- Deliver evidence based interventions as appropriate
- Review effectiveness of care plan at regular intervals
- To work within safeguarding processes, including where required to act as safeguarding investigator / manager.
- To work collaboratively with service users, carers and partner agencies
- To maintain computer held care records in an accurate and timely fashion
- Undertake all work in a culturally sensitive manner
- Act in accordance with Care Management System, Acute and Schedule Care Pathways as appropriate.
- Develop effective working relationships with Primary Care colleagues

- Additional skills /training in psychosocial interventions for people with SMI, CBTp and family interventions
- oRelevant Health or Social Care professional qualification - Nursing or Social Work
- oRelevant Health or Social Care professional qualification - Nursing or Social Work
- oCurrent registration with NMC, HCPC
- oEvidence of continued professional development
- oMentorship qualification
- oSupervision training
- oExperience of multi-disciplinary team working
- oExtensive experience in mental health in both inpatient and community settings
- oExperience of managing/supervising others
- oExperience of working in a recovery focussed way
- oExperience of working with adults in acute/crisis/relapse presentation
- oExperience in a Community Mental Health Team/Assertive Outreach team
- oWorking knowledge and understanding of Care Programme Approach & Care Management
