- To be first point of contacts for patients who are requesting letters/reports
- To liaise with Practice Pharmacist on medication queries.
- To complete record reviews for online access.
- To complete access to health records review in order to process Subject Access Request forms in accordance with GDPR regulation.
- To refer patients to District Nurses where appropriate.
- First point of contact for external contacts such as hospital, ambulance service, community services and care homes
- Communicate with hospitals and other care providers to follow up on patient progression and pathway.
- Liaise with families who have had a recent bereavement to provide them with clear information about the processes following their loss.
- Efficiently manage the organisations email inbox by reviewing, prioritising, and responding to enquiries in a professional and timely manner.

- Educated to GCSE level or equivalent
- GCSE Mathematics & English (C or above)
- NVQ Level 2 in Health and Social Care
- Experience of administrative duties
- Experience of working with the general public
- Experience of working in a healthcare setting
- Excellent communication skills (written and oral)
- Strong IT skills
- Clear, polite telephone manner
- Effective time management (planning and organising)
- Ability to work as a team member and autonomously
- Good interpersonal skills
