As part of the South Integrated Care & Support Team, you'll work collaboratively with health professionals, community partners, and other agencies to provide holistic and proactive care to adults in the Tameside area.
You'll manage a varied caseload and play a key role in:
- Carrying out Care Act assessments
- Completing Mental Capacity Assessments
- Leading and coordinating safeguarding enquiries
- Supporting individuals with complex needs to access the right care and support
- Participating in the team's duty rota and screening incoming referrals
- Promoting independence and wellbeing through creative, outcomes-focused interventions
We're looking for someone who brings professional expertise, compassion, and a strong understanding of legislation and best practice in adult social care.
You must have:
- A recognised Social Work qualification (SW Degree, DipSW, CQSW or equivalent)
- Registration with Social Work England
- Minimum 2 years' post-qualification experience (4+ years preferred)
- Sound working knowledge of key legislation, including:
- Care Act 2014
- Mental Capacity Act 2005
- Mental Health Act
- Human Rights Act
- Confidence managing complex cases, safeguarding, and capacity-related work
- Strong partnership-working skills and a commitment to multi-agency collaboration
